Accident Report Forms

• Operator's Report of Motor Vehicle Accident - Use this form to report a motor vehicle accident. (41k)
• Request for Copy of Crash Report - Use this form to request a copy of an accident/crash report filed with the Registry of Motor Vehicles.This form is screen-fillable, which means you may print and complete this form by hand or complete it on screen and print it. You must print and mail the completed form to the RMV at the address indicated on the form, regardless of the manner in which it is completed. (39k)

Change of Address Forms

• Change of Address - This form is to be used to inform the RMV of a change of address. (59k)

Licensing Forms

• License and ID Application - This application is used to apply for a Written or Road test, convert an Out-of-State license or obtain a Massachusetts ID or Liquor ID Card. (154k)
• License and ID Update Form - This application is used to renew a license, obtain a duplicate or change any information (ie. name or address) on an existing license or ID. (97k)
• Request for Driving Record - This form is used to request a driving record or history for a MA driver. (63k)
• Request for Personal Information Form (Individual or Multiple Records) - This application is used to request personal information from the Registry of Motor Vehicles. (24k)
• Application to Convert Out-of State Driver Education Certificate - Use this form to convert your out-of-state driver education certificate to a Massachusetts driver education certificate. (72k)
• Vision Screening Certificate - This form is used in lieu of the RMV eye exam and must be signed by an optometrist or physician. (23k)
• Walk-in Hearing Request Form - This form is used to request a walk-in hearing on a license suspension or revocation. (14k)
• Application for Intrastate Medical Waivers To Operate Class A,B, Or C, Commercial Motor Vehicles - This form is used to request a medical waiver to operate Class A, B, or C Commercial Motor Vehicles. (46k)
• 7-D School Pupil Transport License Application - Use this form to apply for or to renew a 7-D license. (132k)

Registration Forms

• Disabled Placards and Plates - Special Placards and Plates are available for vehicles that transport medically disabled drivers and passengers. (271k)
• Application for Purple Heart, Veteran Flag, Silver Star and Bronze Star Plates - This appplication is used by qualified veterans to apply for veteran's plates for their passenger vehicles. (55k)
• Vanity Plate Application Form - This application is used to apply for a vanity plate. (26k)
• Lost Plate Affidavit form - This form is used to cancel a Registration without the plates. (30k)
• Application for Military Branch Decals - This application is to be used to request Military Branch and Wartime Service Decals for passenger Veteran (Flag) plates and motorcycle Veteran (Flag) plates. (256k)
• Application for Veteran's Motorcycle License Plates - This application is used by qualified veterans to apply for a veteran's plate for their motorcycle. (50k)
• Application for Antique Motor Vehicle and Year of Manufacture Plates - This application is used to request permission from the RMV to use an acceptable antique or year of manufacture plate. (16k)
• Application for International Registration Plan (IRP) - This application is used to enroll in the International Registration Plan (IRP). IRP is an optional program for licensing commercial vehicles traveling in two or more IRP jurisdictions. A jurisdiction may be a state or a Canadian province. • Nonresident Student Vehicle Information Form - This form applies to students commuting from out-of-state to a school in Massachusetts and students temporarily residing in Massachusetts. Title Forms

• Application for Duplicate Certificate of Title - This application is used to apply for a duplicate title if yours has been lost. (14k)
• Affidavit of Surviving Spouse - This form is used if you, as a surviving spouse, need to title a vehicle that was previously registered to you and your spouse. (65k)
• Title Amendment Form - The Title Amendment Form is used to amend or change information on your title. (20k)

• Application for Owner/Contractor Registration - This application is used to request an owner/contractor registration. (226k)
